We focus on establishing a customized plan of care that is created through an extensive initial evaluation, working in close collaboration with you and other responsible parties.

The guiding philosophy of ESG is that clients should be able to remain at home and avoid institutionalization with the safest levels of independence and quality of life for as long as possible.
As loved ones become disabled and more dependent on the care of family and friends, the responsibilities of assistance sometimes can become overwhelming.

We’re there to provide the most careful and complete assistance required of you or your loved ones, giving you the peace of mind knowing your loved one is well cared for by a compassionate and select group of caregivers.
How the process works
- Extensive Initial Evaluation - ESG schedules an appointment at your convenience with a licensed and registered Occupational Therapist to review your needs and create a customized plan of care. An Occupational Therapist is both a federally and state certified expert in the evaluation and treatment of patients in the accomplishment of self care tasks and activities of daily living (ADL’s). The tasks can range from basic (dressing, bathing, grooming and hygiene) to higher level (laundry, grocery shopping, and other house- keeping activities.)
- Customized Plan of Care - ESG creates a structured plan of care with a detailed breakdown within each area of need to be addressed.
- Plan of Care Implementation - Once the plan of care is determined, a caregiver will begin work for the client. ESG requires that the caregiver provide a daily summary of care provided and client status. These daily notes can be provided to responsible parties if desired.
- Weekly Care Monitoring - Weekly field visits by ESG area managers are conducted to monitor the status of care provided and the satisfaction of the client.
- 24 hour call center for any issues or questions that may arise during term of services.
